If you have already completed the AIMS program and would like a copy of the information given to you at your conference, please fill out the information below. The audio recordings are not kept on file, and you would have left our office with the only copy. 

Please note that it can take up to two weeks to receive your emailed copies of these requested scores.  Please return this form and pay online or print and return with the corresponding payment to our address listed above.
